What documents are needed for enrollment?

To enroll at MOHS, you'll need to have the following documents ready:

  • Parent or guardian photo ID
  • Student's birth certificate
  • Academic transcripts from previous schools (or a homeschool transcript template if applicable)
  • A student photo

Applying for tuition assistance? You'll also need to provide proof of income. Accepted documents include:

  • Recent pay stubs
  • Federal tax returns or W-2s
  • State benefit award letters (showing benefit type and amount)
  • Social Security or disability benefit statements
  • Self-employment records or 1099 forms
